RECOMMENDATION:
Recommendation Concerning Voluntary Vision Insurance - Thomas G. "Tom" Turner, Human Resources Department Director

That the Board take the following action concerning the renewal of the Humana Specialty Benefits Vision Care Contract:

A. Approve and award a two-year Contract to Humana Insurance Company for vision care administration, PD 15-16.042, effective January 1, 2016, for a period of 24 months, with the capability to extend for an additional 12 months at the same rate;

B. Authorize a one-time option for retirees that are presently on our medical or dental plan to enroll in the voluntary vision plan at the group rate, with no cost to the County.  Enrollment for retirees will follow Florida law in that if a retiree stops coverage after their initial enrollment, they will not be able to enroll in the plan at a future date.  Premiums will be deducted either through their Florida Retirement System payroll deduction or Humana will set up a direct deduction with the retiree; and

C. Authorize the County Administrator to sign the Humana Specialty Benefits Renewal letter.

BACKGROUND:
The Office of Purchasing solicited a Request for Proposal. On May 6, 2016, seven proposals were received from from the following:

Aetna Life Insurance Company
Ameritas
Avesis
Florida Combined Life
Humana Insurance Company
Met Life Insurance Company
Reliance Standard Insurance Company

The County has received a two year guaranteed rate for vision care insurance with three, one year extensions with no plan design changes. The network and providers were a key factor in determining the vendor. Following is the breakdown of premiums:

BUDGETARY IMPACT:
There is no budgetary impact.
LEGAL CONSIDERATIONS/SIGN-OFF:
The County Attorney's Office has reviewed the Humana Specialty Benefits Renewal letter and deemed it legally sufficient.
PERSONNEL:
The Human Resources Department will conduct open enrollment meetings and service all employee groups.  We will advise all appointment authorities (payrolls) of any administrative changes in the program.
POLICY/REQUIREMENT FOR BOARD ACTION:
N/A
IMPLEMENTATION/COORDINATION:
A voucher will be the instrument utilized for making payment against the Contract. The Human Resources Department will coordinate with the County Attorney's Office and other appointment authorities to ensure changes are made.
