RECOMMENDATION:
Recommendation Concerning the On-Site Employee Health Clinic - Thomas G. "Tom" Turner, Human Resources Department Director

That the Board take the following action concerning the On-Site Employee Health Clinic:
 
A. Approve a two-year Contract (anticipated to be August 1, 2016, through July 31, 2018) with the option for two one-year extensions, with Concentra Health Services, Inc., for the On-Site Employee Health Clinic; 

B. Develop a Pharmacy component to assist in the care of Escambia County and Sheriff’s Department employees, upon approval of a two-year Contract with Concentra Health Services, Inc., for the On-Site Employee Health Clinic; 

C. Eliminate the Health Specialist position, change the part-time Medical Assistant position to a full-time position, and upgrade and promote the Nurse Clinic Manager position; 

D. Shift the cost sharing of the Clinic to the County paying 75% of the cost and the Sheriff paying 25% of the cost of the Clinic; 

E. Upgrade the current electronic medical records system; and 

F. Authorize the Chairman to sign the Contract with Concentra Health Services, Inc.  
BACKGROUND:
The Escambia County Health Clinic Agreement was signed in September 2010, with the Clinic opening in January 2011. Per the Contract, the Agreement was extended yearly. In 2015, it was determined that a feasibility study should be conducted. AON Hewitt conducted the feasibility study and it determined that the Clinic should be retained and the two parties should negotiate a new Agreement. The cost of the first year of the Agreement is estimated to be $1,061,892 and the County portion will be $796,419. There is a 3% increase each year for staffing fees.

Changes in the Contract include:

1. Eliminating the Health Specialist Position.
2. Adding a full-time Medical Assistant.
3. Upgrading the Nurse Manager position.

With the new Agreement, the Clinic will update their medical records system to assist the County in tracking trends and provide more secure options for the electronic medical files. This requires an additional cost of $12,555 per year for licensing and a one-time fee of $7,125 for installation.

In the Agreement, there is a component to develop a pharmacy element in the Clinic that will disperse non-narcotics to patients. This will provide ready access for patients to receive medication immediately and return to work sooner.

Due to the switching of the Jail employees to the County and the addition of the Library, the Cost sharing will be 75% the County's responsibility and 25% the Sheriff's responsibility.

As the County has moved to Self Funding Health Care, the Clinic will provide a greater opportunity to manage our health care costs and develop programs that will better assist employees with health care issues. The continuity of providers will be critical in keeping the trust that people expect in health care. In the future, the self-funded plan will permit us better claim information to evaluate the clinic's value.
BUDGETARY IMPACT:
Funding for the On-Site Clinic is through Fund Code 501, Internal Service Fund.

IMPLEMENTATION/COORDINATION:
Upon Board approval and legal review, the Human Resources Department and Concentra will execute the Agreement.
The Human Resources Department will coordinate with Concentra and the County Legal Department.
