That the Board approve and authorize the County Administrator to execute the following Change Order #1, to Bound Tree Medical, LLC, for medical supplies for Emergency Medical Services:

BACKGROUND:

In its meeting held January 7, 2016, the Board authorized issuance of blanket purchase order to Bound Tree Medical LLC., in the amount of $52,500 for purchase of medical supplies for Fiscal Year 2015-16. The recommended Change Order will add funds in an amount to total that estimated to be needed for the balance of the current fiscal year. The increase is necessary due to higher call volume and new procedures put in place by the Medical Director.

Funding is available in Fund 408, Emergency Medical Service Fund, Cost Center 330302, Object Code 55201

POLICY/REQUIREMENT FOR BOARD ACTION:

This recommendation is in compliance with the provisions of The Escambia County Florida Code of Ordinances, Chapter 46, Finance, Article II, Purchases and Contracts.

IMPLEMENTATION/COORDINATION:

Upon approval of this recommendation, a Change Order will be transmitted to the Office of Purchasing for processing.
