BACKGROUND:

On October 24, 2018, a Purchase Order was issued to Pensacola Concrete Construction Co., Inc., in the amount of $44,409.05, to perform a point repair to a small section of deteriated stormwater pipe within the right-of-way adjacent to 6694 Hallendale Drive in the Crystal Creek Subdivision, and then to rehabilitate the pipe with cast-in-place pipe liner. Quotes (#10022018) were received utilizing the Vendor Registry website.

During construction, an additional 50 linear feet of pipe collapsed with a portion being under a driveway. Camera work was performed from each end of the pipe up to a blockage. The extent of the void space around the blockage could not be seen during the camera work.
This Change Order, #1, in the amount of $14,942.30, adds additional work required to make the necessary repairs. Work includes but is not limited to replacing an additional concrete driveway, replacing adjacent curbing, dewatering, by-pass pumping, and additional sodding.

This project is located in Commission District 1.

Funds are available in Fund 175 Transportation Trust Fund, Cost Center 210402, Object Code 54601, Project Number N/A

POLICY/REQUIREMENT FOR BOARD ACTION:
This recommendation is in compliance with the provisions of the Code of Ordinances of Escambia County, Florida, Chapter 46, Finance, Article II, Purchasing and Contracts.
IMPLEMENTATION/COORDINATION:
Upon approval of this recommendation, a Change Order will be transmitted to the Office of Purchasing for processing.
